I have posted this caution before, but let me do it again. In my
experience, the use if the EditCitation feature on the RefMan toolbar to
delete references sometimes will cause all of the references below the
one being worked on to shift in position, requiring reinserting them all
manually.
This was a major problem in 10 & 11, but the ver. 11 patch improved it.
HOWEVER, I had it happen once with the patched version (while polishing
a grant during the last few hours), and after this, I now delete
manually (highlight only the citation of interest, use the
Revert-to-Text button, and manually delete the reference I wish to
eliminate, then regenerate the bibliography). It may be that this bug is
now completely fixed, but it is a disaster when it hits.

Hello Jenny,
It might be one or both of the following:
(1) When you delete references, it's best to use the Edit Citation(s)
feature on the toolbar, then delete the rest of the text. Highlight the
reference, click on Edit Citation(s), then select the reference you want
to delete & click on Delete.
If you are deleting references the regular way, the order can be ruined.
(2) Sometimes the first reference appears as (2) if there is a reference
hidden in a comment or textbox somewhere. See the RefMan FAQ here:
http://www.refman.com/support/faqs/CWYW/faq12.asp
I hope this helps!

Subject: <RefMan> deleting references
Hi all,
I am using reference manager 11 with word, cite as you write for a long
document. When I delete a chunk of text with a reference contained in
it, the references do not re-order themselves and the deleted reference
remains in the reference list (even when list is re-generated). So, for
example, my first reference is currently numbered (2) and the reference
list has an references from a previous draft as (1)
Any body had the same problem / know any answers?
Thank you
Jenny
