I second the advice of turning off Track Changes when using Instant Formatting and/or vice versa, as Refman does not seem to do deal with the hidden text very well. I usually use Track Changes to come up with an agreed upon versions, then accepted all changes, and run the Bibliography with Track Changes turned off. Then repeat for the next version. If I have to use Track Changes, I usually disable Instant Formatting, too. Somehow the Track Changes feature seems to kluge the document up along the way, and I get unexpected results from RefMan (like having the in-text citations shift forward). Also, you have to go through about 50 steps of Undo if Instant Formatting kicks in when you make a change that you then want to undo.
