This [N,M] is not an error code, rather it indicates that inside your programme you have ended up with a matrix(vector) inside your main matrix.
A simple example would be using the lookup function, which can find more than one value. And, as the descriptrion, says "When multiple values are returned, they appear in a vector"
If you select the specific element (e.g. one that says [N,M] ) and ask for that value to be shown ( Matrix [ i,j = ) you should see it is an NxM matrix(vector).

When calculating a matrix or vector, will sometimes get two numbers in
brackets in each element of the matrix or vector. They are related to
errors in the program. So this appears to be some sort of error code.
Does anyone know of a catalog or list of the these??? It would come in
handy for debugging.
Also the error message list in Mathcad 11 Help needs updating. Many
error messages are not listed at all (usually the ones I run into), and
some are worded wrong.
NOT eagerly awaiting Mathcad 13.

These aren't error codes, these are indications that you've created a
nested matrix. If the nested entries are sufficiently small, you can
display them by double-clicking on your result, and choosing "expand
nested arrays" on your display tab.
Otherwise, you can pull out entries in the matrix by evaluating them
with the subscript [ operator to take a look at their contents.
