Dear All,
Before I try to write one, does anyone know of an output style for this
journal, or of another journal that uses the same style? We have
EndNote 9.

Try "Academic Medicine". I think the main changes will be to
unsuperscript the citation and put it in square brackets and to put a :
after the authors. Also list all author names in the bibliography Author
lists setting. I obviously started with the "A"s.
