With Zotero, there's an automatic date added feature, and so all I do is
import to EN.

My PubMed connection file changes JAMA to Jama, BMJ to Bmj, etc.
I went to edit the connection file, where in the Field editing
box you
can instruct the connection file on how you want it to handle
capitalization. But I was surprised to see that there isn't a
journal
field, and I couldn't figure out which of those listed
corresponds to
the journal field.

The generic field corresponding to Journal is "Secondary Title".
The setting should be: Do Not Lowercase
