I have faced similar problems of trying to wrestle citations in a
dissertation into what seem to be unanticipated formats. I finally
surrendered and had Endnote cite them however it wanted, then in the
final draft, when the text is decoupled from any automatic updates, I
rendered a universal document search and replace. In the instance of
"Smith (Smith, 2004)", this would look like:
Find: Smith (Smith, 2004)
Replace: Smith (2004)
