I have been searching the web, archives, and the user manual to no avail on the following problem. I posted this question to the help desk at ISI ResearchSoft as well, and haven't heard back from them yet.
I would like to create an EndNote output style that allows a notation to be added to the author field, linking it to the author address field. I would want this to function much like many journal articles do--when each other is listed, a number or ** follows each, with the number or ** explained below the author list, indicating where the author works. An example would look like this:
"This is my title" Beth Allen* and Somebody Else**
* Beth's location
*Somebody's location

I know this is available in many of the document templates available with EndNote 7.0. However, I'm trying to use data that is contained in an EndNote library to create a list of citations, and have the author address indicated for each author, using a notation of some form. I can't find that this is possible as an output option. I'm not sure why it wouldn't be--if the EndNote document templates allow for address location as part of their styles, why wouldn't the output style area in EndNote allow this?
Does anyone know how to create an output style that allows the notation information like I'm describing above? If so, please help!

Unfortunately, Endnote allows only one address (similar to the way
Pubmed deals with information-one per record) and the address is not
linked to each author. Hence, I don't see how you would specify this.
Endnote is not a relational database. Manually you could use the custom
fields and maybe even unused fields, but it would require you to include
the superscript as a part of the author name and manually construct the
style to include the various fields in the appropriate place. You are
correct, it does allow this kind of thing, one to one, when you author a
paper- thru the templates, but I suspect very few (even powers users)
use this feature. It is clear from the response to a query I had, that
NO ONE uses the insert figure features. (maybe that will generate a
response).
Endnote is, first and foremost, a program designed for a specific
purpose-annotating the bibliography in the myriad of ways journals
demand. Yes, clever people have used it for other things, but that is
not its function. Most of the complaints about endnote have probably
arisen because the program gets bigger and bigger trying to be all
things to all users.
