I use En.8.0.2
My Do Not Change Case list is a text file located at C:\Documents and
Settings\<USERID>\Application Data\EndNote\EnCase.txt
Editing this file directly (1 entry per line) flowed through to the relevant
page on the En Preferences dialog. (Whether the feature actually works is a
different question, and I don't know the answer).
In writing this message, I tried putting some text into that file in Greek
to see how it would handle Unicode. En will read an ASCII or a UTF8 .txt
file, but not what windows notepad calls "Unicode" or "Big Endian". (You can
check the encoding by opening the file in notepad and going file/save as.)
I think the Change Case file has been outside the registry for some time,
but I have no way of checking here.

What version of endnote? In endnote 7 and earlier, it is probably
buried in the Registry. If endnote 8.02 (there was a 'case list' bug in
8.0) or 9, it should be in the preferences file. Having said all that,
I couldn't find any of the words in my ignore list, anywhere in the
registry, when I searched. (I still use Endnote 7)
You really need to contact Tech support, who should be able to help you.
There has been a report of this previously in the archives of the
interest group list (Endnote7, MacOS, Nov, 2004), but no helpful
responses.
