Here's what I do:

1) Perform your search in PubMed.

2) Send the citations you want to the clipboard.

3) When you are ready, view the Clipboard.

4) Select "Medline" in the Display tab.

5) Then click save.

6) Save the file to your disk, making sure you select All Files as File
type. Out of habit I name the file with a .txt extension.

6) In EndNote, select File ---> Import.

7) Choose PubMed as the Import Option, and click Choose File. Browse your
way to the directory in which you saved your PubMed search.

8) Click Import.

9) you're done.

I hope this helps.

Find the connect command (I think it is under File, in Endnote5) select
pubmed from the long list (next time it will be the "default" choice in the
menu), and use the endnote search window interface to find records you want.
then drag and drop to your library, or copy them to a new library to weed
thru, annotate and (file menu?) import (with the option to throw away
duplicated, if you like) into your library. If you select one record or
more, the bar at the top gives you the option to copy the records directly
to your library or to a new or different library. If you have trouble
getting the connect command to work, you might be trying to go thru a proxy
and that is another story.
