Shortly after posting a problem here, I found the solution. In brief, I
had a Word document that referenced a deleted citation in my EndNote
library. If I searched for the citation in EndNote directly, I found
only the valid entries, but if I did the search from within my Word
document (magnifying glass icon), I saw an additional entry that was
bogus.

The solution was to delete ALL references to the bogus entry. I'd tried
deleting (and replacing) one such reference, which didn't fix the
problem. Turns out there were multiple citations -- when all were
removed, the bogus entry also disappeared.
